Upon Adding of User duplicate users displayed in users list #3785

After getting past the error previously mentioned in the previous post "Passwords must be at least 3 characters." you will see the attached image result. Duplicate users displaying in the users list. If you refresh you will see the list get "cleaned up". But its not nice to see duplicate users in your user list when there should'nt be.
