How do we add these to the AppMenuItem definitions?

This is currently not supported. You can create an issue here <a class="postlink" href="https://github.com/aspnetzero/aspnet-zero-core/issues">https://github.com/aspnetzero/aspnet-zero-core/issues</a> and we can implement it for the next version.
